D4 - Co-Creating Transition System Change Efforts (CE Offered)

People with Intellectual and Developmental Disabilities may need extra support to live full, meaningful, and connected lives which can make the transition to adulthood complex. Staff and partners with lived experience in disability from Wisconsin will describe how a project uses both “top down” and “bottom up” approaches to improve and integrate school/work/health transition support systems driven by community voice. Participants will learn about the Community of Solutions model including the intentional collection and sharing of stories, building skills and trust, and what it looks like in urban and rural areas. They can see video examples and tools, hear lessons learned and what we continue to struggle with, and be invited to share their ideas and strategies.
