(VP17) Development of a CYSHCN Case Management Practice Model: Process and Partnerships

The Texas DSHS partnered with a university institute to develop a comprehensive case management practice model that is trauma-informed, strengths-based, family-centered and responsive to national standards for serving CYSHCN. The process of developing the practice model involved on-going partnerships with key stakeholders at each step of the project that included a state-wide needs assessment to understand CYSHCN population and services, advisory groups with staff and caregivers during the development of modules, and in-person and virtual implementation of the training state-wide.
