D2 - Keeping the P’s: Purposeful Partnership in the Health Equity Zone Initiative

Rhode Island’s Health Equity Zone Initiative creates a network of diverse, cross-sector, and resident-led collaboratives across the state of Rhode Island that address social determinants of health in effort to achieve health equity. The Rhode Island Department of Health partners with equity-driven agencies, residents, healthcare providers, social service providers, and other state agencies in order to support the HEZ Initiative. The success of the initiative rests in our innovative model, which uses a personalized, power-building, and trust-building approach. In this workshop, we will share the key components of our model that allow for the development of authentic, transformational relationships with community partners.
